Individual

The cost of membership for an individual is:

$95 per month, or

$285 per quarter, or

$570 every 6 months, or

$1045 per year

For new patients, there is a one-time enrollment fee of $95.

Couple

$180 per month, or

$540 per quarter, or

$1080 every 6 months, or

$1980 per year

For new couples, there is a one-time enrollment fee of $180.

Family

$270 per month up to 4 people. Each additional child, add $10

$855 per quarter up to 4 people. Each additional child, add $30

$1710 every 6 months up to 4 people. Each additional child, add $60

$2970 per year up to 4 people. Each additional child, add $110

For new families, there is a one-time enrollment fee of $200.

(A family is defined as parent(s) and unmarried children under the age of 26 residing in the same household.)
